OSHA Outreach Training Card Request

10 Hr & 30 Hr Outreach Card Request

Thank you for attending the training at the Region VII OSHA Training Institute Education Center at the MCC-Business & Technology Campus (MCC-BTC) in Kansas City.

For detailed instructions on the Department of Labor's Construction or General Industry Outreach Training Program Guidelines, refer to the following:

In order to submit your request for cards to us, you must have:

• Attended the OSHA 500/502 courses at MCC-BTC,
• Attended the OSHA 501/503 courses at MCC-BTC,
• Received training within the last 4 years, or
• Received training through MCC-BTC 360Training Course (Internet) in OSHA 500 or 501.

2. List of attendees you trained
